HOW TO BECOME A PATIENT
Your first trip to Hope Community Clinic is simple…
You’ll meet staff who are friendly and easy to talk with. After you complete some general paperwork you’ll meet someone who will ask you questions that will help them provide you the best options .
GET YOUR NEW PATIENT APPLICATION
Click “Become a Patient” to fill out our online application, download an electronic copy or pick up a printed copy our clinic. Complete an application for each new patient, even if they’re in the same family. Once completed, someone from our front desk will review. Don’t forget to include identification and proof of income.
SCHEDULE YOUR FIRST APPOINTMENT
We will call to find a time that works for you and your family. Make sure you write down your appointment date and time!
COME SEE US!
Make sure you bring all your medications, your insurance card if you have one, and any pieces that were missing from your application. We look forward to welcoming you to Hope Community Clinic!
